Should you need to apply for Financial Assistance, you must complete the attached form, and contact the Registrar for further registration information. Registrars@southlandhockey.com

 The Flames Financial Assistance Program has recently changed its name to the Flames Even Strength Program.

  If applying for financial assistance, please ensure that you have the completed registration form, along with the appropriate documents.  You MUST now contact the Registrar at registrars@southlandhockey.comin order to register your child with the Financial Assistance program and SHA.

  Incomplete applications will not be submitted to Hockey Calgary.  

   Should you need to apply for Financial Assistance, please bring copies of the following paperwork with you to attach to the application:

 ·         PLEASE SUBMIT ONE OPTION FROM EACH SECTION (One and Two) AND INCLUDE A COPY OF EACH WITH YOUR APPLICATION (required)

·         Section One

Employee Pay Stubs: Copies of THREE of your most recent pay stubs.

Income Support Stubs: Formerly known as Support for Independence (SFI)

Assured Income for the Severely Handicapped: (AISH) stubs

Workers Compensation Board: (WCB) pay stubs

Employment Insurance: EI stubs

Alberta Child Health Benefit Card: Please send in a photocopy of the card and THE LETTER that verifies your length of coverage. NOTE this is not your Alberta Health Care Card

Canada Child Tax Benefit Notice: CCTB for the current year showing family income and marital status

Notice of Assessment: Only the most current Notice of Assessment for the prior tax year will be accepted. This is the form you receive from Revenue Canada after filing your tax. Require Notice of Assessment for BOTH PARENTS.

 All applications must be completed in full or they will not be submitted to Hockey Calgary for review.   

If you have applied for financial assistance previously, please note the following regulations:

1st year application – covered 100% to a maximum of $600.00

2nd year application – covered 75% to a maximum of $450.00

3rd year application – covered 50% to a maximum of $300.00

 

 Please note that you are NOT guaranteed financial assistance, and any funds that are due after the above amounts are deducted from the registration fees are due at the time of registration.  Should additional funds be provided to you, a refund will occur once funds are received from the Flames EvenStrength Program Program.
